MARCH 23, 2018  On March 23, 2018, staff were invited to participate in person or on the Departments’ Facebook page as the Department “pushed the final submit button” to the Public Health Accreditation Board (PHAB). From October 22-23, 2018, a Site Visit Team will be coming to San Bernardino County to verify elements of documentation submitted by the health department, inspect our facilities, conduct multiple interview sessions, and learn more about the department’s governance and community partnerships to ensure they have an accurate and complete picture of the department and how it conform to PHAB measures.

JUNE 1, 2017  On June 1, 2017 the Department successfully uploaded three (3) documents to the e-PHAB system! Several other documents are in progress and will be uploaded very soon.

JANUARY 6, 2017  On January 6, 2017 the Department submitted our application to PHAB. This has officially lead us into Step 3 of the Accreditation Process – Document Submission and locked us into Version 1.5 of the PHAB Standards and Measures. The Department has 12 months to submit documentation that demonstrates our Department’s conformity with PHAB Standards and Measures. It’s going to be an exciting year ahead!
